Transitions of Care: A Growing Opportunity for the Community Pharmacy

Nearly 20 percent of hospitalized Medicare patients are readmitted to the hospital within 30 days of discharge. Not only do these re-hospitalizations cost the system a huge amount of money, they also cause undue stress to patients and their caregivers. Community pharmacies are ideally located to assist in the transitions between care settings. This session will highlight the opportunity and help pharmacies interested in this service detail a plan, including how to get paid for the service.
